Cornerstone VNA is proud to announce that Health Recovery Solutions (HRS) has awarded Anne Carle, RN, Telehealth Clinical Coordinator with the 2019 Telehealth Honors® Award. The awards program, which is celebrating its 3rd year, recognizes those individuals and programs who provide exceptional patient care in the home using modern telehealth technology.
The Telehealth Honors® are the first awards in the nation to showcase outstanding stories of successful patient care using telehealth in home based care including home health, hospice, palliative care, assisted living, and acute care transition models. Winners are a testament to how modern telehealth solutions have enriched the lives of those involved while supporting the initiatives of the providers. Recipients of the award were selected by judges in the Home Health and Telehealth industry. Carle has received the Telehealth Honors® every year since its inception in 2017.
According to Jennifer Gullison RN, MSN, Chief Clinical Officer at Cornerstone VNA, “Anne finds creative and new ways to utilize home Telehealth remote monitoring. She is passionate and puts her heart and soul into this program.” Gullison adds, “Anne is devoted to Cornerstone VNA’s Telehealth program and to utilizing all of its functionality for the betterment of our patients. This program would not be what it is today without her.”
Cornerstone VNA has been offering Telehealth services since 1998 and upgraded to a more sophisticated program in 2016 with HRS. Although Telehealth is not currently reimbursed by health insurance, Cornerstone VNA believes in the technology and is committed to providing this service to high risk patients at no charge. Telehealth provides significant benefits to patients and has proven to reduce hospital readmissions. In 2018, the 30-day hospital readmission rate for Cornerstone VNA Telehealth patients was 5.4%, which is significantly less than the national and state readmission rates of 12.2% and 11.6% respectively.
