Palliative Care
Palliative care is specialized medical care with the goal of improving the quality of life for the individual and family of those living with serious illnesses. Our palliative team of nurse practitioners (APRNs) are experienced and able to provide:
- Consultation, recommendations or medical management of symptoms associated with life-limiting disease processes such as COPD, Congestive Heart Failure (CHF) or cancer.
- Assistance to patients and families with navigation of goals of care in a complex medical system.
Team
Our clinical staff in the Palliative Care department hold Master’s Degrees in nursing. We’re proud to have Nurse Practitioners advanced certifications in hospice and palliative care in our team.

Eligibility
- An individual may qualify for Palliative Care if they have a life-limiting disease such as chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), or cancer.
- Unlike Hospice, Palliative Care may be utilized while an individual is still seeking curative treatment or is not yet at the end of their life.
- An individual does not need to be homebound to qualify for Palliative Care.
- Requests for a Community Palliative Care consultation by a nurse practitioner (APRN) must be made through a referring provider. Contact us to learn more.

What is Palliative Care?
Palliative Care is specialized medical care for people with a serious illness, such as such as chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), or cancer. Patients can receive Palliative Care services while still receiving curative treatment for their disease. Palliative care is not Hospice care and is not limited to end of life.
How can Palliative Care help me?
We focus on providing patients relief and medical management of symptoms due to a complex illness. Our goal is to improve the quality of life for you and your family, and decrease the need for emergency room visits or re-hospitalizations.
Do I have to be homebound to be a Palliative Care patient?
You do not need to be homebound to receive this service, and care is provided wherever you call home, such as a private residence, assisted living facility, or nursing home.
What do I need to do to become a Palliative Care patient?
We encourage you to talk to your physician about a referral to our Palliative Care program. You can also contact us to learn more about our program and find out if Palliative Care is right for you.
