Medical Social Work

Medical social workers complete psychological assessment, develop discharge plans, arrange home care services, provide appropriate referrals, offering support and advocacy for patients and their families.
Team
Medical Social Workers, also known as a health care social worker, assist patients and families in dealing with diseases. They assist patients in understanding their sickness or condition and offer information on the resources available to help them manage with the emotional, financial, and social requirements that occur as a result of the diagnosis. A social worker serves as the patient’s advocate, ensuring that their emotional and social needs are satisfied through medical therapy. We have several Social Workers in both our Home Care and Hospice teams.
Some common responsibilities and duties of a medical social worker include:
- Reviewing new admissions for any special needs or issues that need to be addressed
- Assisting with patient discharged from a hospital, clinic, or nursing home setting to return home
- Coordinating the care of patients with complex needs that require multiple services over time
- Handling crises that come up unexpectedly with counseling or therapy
- Helping patients understand the financial and emotional resources available to them
