This type of care is often required for individuals recovering from surgery, illness, or injury, or those with chronic conditions that require specialized medical attention. Skilled nursing services can include administering medication, wound care, physical therapy, intravenous therapy, and other complex medical treatments.

Home health nursing focuses on providing medical care and support to patients in the comfort of their own homes. The goal is to promote healing, manage chronic conditions, and improve the quality of life for those who hare homebound and do not need to stay in a hospital or care facility.  Home health nurses typically:

  • Monitor and assess health conditions: They check vital signs, manage wounds, and evaluate progress in recovery.
  • Administer treatments and medications: This can include IV therapy, injections, or managing medical equipment.
  • Educate patients and caregivers: Nurses teach families how to care for their loved ones, whether it involves handling medication schedules or understanding symptoms to watch for.
  • Coordinate care: Home health nurses often work alongside doctors, physical therapists, and other professionals to ensure patients receive comprehensive care.
  • Offer emotional support: Building rapport with patients and families is key, especially during challenging times.

Services are performed by Registered Nurses and Licensed Nursing Assistants (aka Certified Nursing Assistants) in conjunction with the PCP or Specialist Providers, as well as our Rehabilitation staff and Medical Social Workers, where indicated.

Who Can Receive Services?

  • You have to be under the care of a physician while you are on home care services.
    • The physician confirms in writing that you wer examined within 30 days and need skilled nursing care. The doctor can be a hospitalist or community-based doctor.
  • You require the skills for a nurse or therapist in the home. Most insurances require you to be homebound. This means:
    • You have an illness/injury that restricts your ability to leave their residence without the aid of supportive devices or special transportation. You may also be considered homebound if you require the assistance of another person to travel or if leaving home would cause your illness/injury to worsen.
    • It is taxing and hard for you to get out on a routine basis.
    • You need the assistance of a person, device, or special transportation.
    • You leave the home infrequently for things other than for medical reasons.
    • You only need short-term or part-time skilled services.
